Healthcare transformation with a direct impact on patients
‘El Meu Clínic’ not only improves access to information, it also acts as a driving force for innovative projects such as:

Home hospitalization: it allows for real-time patient monitoring with monitoring systems that warn of signs of clinical deterioration, video calls and asynchronous communication channels. The smart alert system activates scalable care and transfer protocols, with automatic recording in the patient’s electronic medical history.

Type 1 diabetes: it incorporates sensors and hybrid closed-loop systems for remote monitoring and control. The tool prioritizes access to consultations according to the degree of metabolic control (classified into 5 levels), and sends quarterly reports to the patient. The model has already been tested on more than 350 patients and is now being extended to 2,500 users of Abbott and Medtronic systems. Projects are also underway in the fields of outpatient pharmacy, endometriosis and cardiac tele-rehabilitation.
New projects being developed and future features
New lines of action are being developed, such as the monitoring of liver transplant patients, smoking cessation, preparation for tests, support for pregnant women and newborns at home, non-invasive ventilation, heart failure, and home haematological care.
At the same time, new features will be added such as the visualization of surgical schedules, and appointment management has been consolidated: in the last month alone, 777 appointment reschedules and 155 cancellations were managed through the platform.
Towards a scalable, patient-centred model
‘El Meu Clínic’ is a scalable tool, designed to support healthcare processes and focused on the patient experience. It includes questionnaires (PROMs), educational material, structured reports and direct access to clinical services, reinforcing a more participatory, efficient and results-based model. In short, ‘El Meu Clínic’ is consolidating its position as a secure connection space aimed at all patients with an active medical history number at the Hospital Clínic Barcelona. It is a project promoted by the Strategy and Planning Management, the Information Systems Management, and the Citizen Services Area.
