Organization - Geriatrics Department Service
The Department has a territorial vision and establishes close coordination with intermediate care and primary care, including nursing homes and home care programmes. This care network facilitates a proactive model that combines prevention, treatment, rehabilitation and, when necessary, palliative care.
The Geriatrics Department currently consists of six specialist consultants, a Head of Section and a Head of Department. It has nursing professionals and nursing assistants, as well as other professional and other profiles such as social workers, physiotherapists, nutritionists, clinical psychologists and administrative support.
Geriatrics Department Facilities
The Geriatrics Department has various facilities to meet the needs of patients. These include an Acute Geriatrics Unit, Hospital Support Teams, an Interdisciplinary Outpatient Department, a Day Hospital (cross-disciplinary) and an Orthogeriatrics Unit. Each facility has a specific objective to provide comprehensive care to geriatric patients.
Hospitalization wards
Acute geriatrics unit:
It primarily cares for patients over 85 years of age and other elderly adults based on criteria related to their acute illness, baseline condition and complexity:
Orthogeriatrics unit
Provides coordinated care with the Orthopaedic Surgery and Traumatology (OST) Department for patients with hip fractures.

Liaison nurse:
Cross-specialty geriatrics:
Consultation with cardiology to assess geriatric patients who are candidates for the implantation of different valve devices (TAVI) or pre-surgical assessment of elective treatment of aortic aneurysms.
Outpatient activity
GERIATRICS CONSULTATION:
Monographic lines of orthogeriatrics and frailty
Consultation with the advanced practice nurse (APN):
Specific lines on heart failure and fall prevention
NON-ONCOLOGICAL PALLIATIVE CONSULTATION:
Specific cross-disciplinary line of care for non-oncological palliative patients
Responds to queries, performs comprehensive geriatrics assessments, case management, coordination with other levels of care, and care and follow-up of patients with complex and advanced chronic diseases.
